Molina Healthcare 2022 Enrollment Bonus 

Bonus for New Agents 

• 25-49 new members – $30 per member 

• 50+ new members – $55 per new member 

Payment Details:

• Bonus payment will pay out in June 2022 to your agency. Your agency is solely responsible for paying any bonus amounts to you

• Bonus amounts must be earned by each agent individually. New enrollments by an agent will not be added to enrollments by a different agent for purposes of meeting the enrollment target

• One-time payment. [Not Per Member Per Month (PMPM)]

 NEW MEMBERSHIP ONLY!

• Members must effectuate 1/1/22 or 2/1/22

• Members must remain enrolled with Molina through 3/31/22 for 1/1/22

effective membership or 4/30/22 for 2/1/22 effective membership

• Members must not have any data discrepancies in the 834 report

• Members cannot be in grace period for nonpayment of premium through

3/31/22 for 1/1/22 effectives or 4/30/22 for 2/1/22 effective membership

Bonus for Renewing Agents 

• 20+ overall members – $30 per member 

Payment Details:

• Bonus payment will pay out in June 2022 to your agency. Your agency is solely responsible for paying any bonus amounts to you

• One-time payment. [Not Per Member Per Month (PMPM)]

• The above table outlines the amount of renewed members enrolled and the amount per member that you will earn

• Bonus will be based on book of business (BOB) through 3/31/22 (or 4/30/22 for 2/1/21 effective membership)

RENEWING MEMBERSHIP ONLY!

• Members must effectuate 1/1/22 or 2/1/22

• Members must remain enrolled with Molina through 3/31/22 for 1/1/22 effective membership or 4/30/22 for 2/1/22 effective membership

• Members must not have any data discrepancies in the 834 report

• Members cannot be in grace period for nonpayment of premium through 3/31/22 for 1/1/22 effectives or 4/30/22 for 2/1/22 effective membership.
